Experienced Home Healthcare LPN – Office-Based Care Coordinator
Pay: $33.00 - $35.00 per hour
Job description
We are seeking an experienced, compassionate, and dependable Licensed Practical Nurse (LPN) with a strong background in home healthcare to join our team in an office-based care coordination role.
This is primarily an office-based position and does not involve regularly providing hands-on care in clients’ homes. The LPN will work under the supervision of an RN and assist with coordinating and managing client care. This role is ideal for a nurse who understands the unique needs of home healthcare clients and is comfortable communicating with clients, caregivers, nurses, families, and office staff.
Responsibilities
- Assist the supervising RN with coordinating and managing client care
- Monitor client needs and communicate changes or concerns promptly
- Maintain accurate and timely client documentation
- Communicate professionally with clients, families, caregivers, and healthcare professionals
- Assist with care coordination, client follow-up, and staffing needs
- Support caregivers and field staff with client-related questions
- Address client and caregiver concerns in a timely and professional manner
- Communicate clinical concerns and changes in client condition to the supervising RN
- Follow company policies, procedures, and applicable healthcare regulations
- Maintain client confidentiality and professionalism at all times
- Perform duties within the LPN scope of practice under RN supervision
Weekend On-Call Requirement
This position includes participation in a rotating weekend on-call schedule.
When assigned, the LPN must:
- Carry the company on-call phone
- Remain available and responsive to calls
- Assist with client-care and staffing concerns
- Communicate clinical concerns to the supervising RN
- Respond calmly, professionally, and promptly to clients, caregivers, and field staff
Qualifications
- Current and valid Licensed Practical Nurse (LPN) license
- Previous home healthcare experience required
- Strong understanding of home healthcare client needs and care coordination
- Excellent communication and organizational skills
- Strong documentation and follow-up skills
- Ability to communicate effectively with clients, families, caregivers, nurses, and healthcare professionals
- Ability to work independently while collaborating closely with the supervising RN and office team
- Professional, dependable, compassionate, and responsive
- Ability to participate in the rotating weekend on-call schedule
